Incident Reports

 

 

I. Scope:

 

This standard establishes requirements for the preparation of reports for incidents to which the department responds.  This standard was formed to:

A.   Create a permanent record of each incident to which the department responds.

B.    Develop a database for the analysis of the response area’s demand for fire and emergency medical services.

C.   Provide uniform data to the department, the Town of Vegreville and as requested by the County of Minburn, concerning the department’s emergency response activity.

 

II. Completing the Report:

 

A.   The department uses the incident reporting system formed by the Vegreville Fire Department.

B.    An incident report shall be completed for each incident to which the department is dispatched and the report shall remain filed with the department.

C.   The report shall be accurate and thorough and it shall contain sufficient information to allow the reader to recreate an accurate portrayal of the facts and events surrounding the incident.

D.   The dispatch office maintains a chronological number for each incident and the member filing the report shall record the appropriate incident number on the report.

E.    Blank report forms shall be carried on all in-service apparatus so that information may be gathered at the incident scene.

F.    Whenever an incident is being reported to the Fire Commissioner’s office the member completing the report shall fill out the appropriate forms used for the Commissioner’s office and submit them to the Fire Chief.

G.   The proper number and types of vehicles, the total manpower and the equipment that is used shall be entered on each incident report.

H.   The member filing the report shall report all pertinent information in regards to a Motor Vehicle Collision.  All vehicle registration, insurance and driver information shall be obtained at the incident scene, if possible.

 

III. Responsibilities:

 

A.   The officer/member of the command vehicle or if not available, the officer of the first arriving vehicle shall be responsible for the completion of the report.

B.    Reports shall be signed prior to submitting to administration.

C.   The Fire Chief shall be responsible for causing the creation of a monthly report that provides an analysis of incident activity for each month.
