Repair Requests
I. Scope:
This standard establishes a record-keeping system for repair requests. It was created to:
A. Provide a record of requests of repairs needed for apparatus, equipment and station deficiencies.
B. Provide a record of repairs done in regards to the request made.
C. Maintain all apparatus, equipment and station operations in a constant state of readiness.
II. Reports:
A. The repair requests records shall be maintained by the Fire Services Coordinator.
B. Records shall be located in the apparatus deficiency/repairs binder which is maintained by the Fire Services Coordinator.
C. Records will be reviewed on an annual basis.
D. The supplied Apparatus/Equipment Deficiency Reports will be used for recording purposes. These reports are located in the administration area.
E. When filling out an Apparatus/Equipment Deficiency Report, the following information is required:
1. Incident Type or Number – If the deficiency was noted during an incident, the Incident Number will be listed. If the deficiency was noted off duty or at other times, N/A will be used.
2. Apparatus/Equipment Involved – Indicate the apparatus involved, or a brief description and location of the piece of equipment involved.
3. Action Required – Briefly describe the deficiency and the possible action required to correct the problem.
4. Action Taken – Describe what you have done before submitting the report.
5. Recommendations – Recommend what needs to be done and if possible, what can be done to lessen the chance of this deficiency happening again.
6. Signatures – The member filling out the report shall print and sign their name in the appropriate place. Should there be any questions about the deficiency the person who filed the report can be contacted for further information.
F. Reports shall be submitted to the Fire Services Coordinator who will in turn forward the request to the Fire Chief or in his absence the Deputy Chief, for approval of repairs.
G. When repairs or corrections have been made, the Fire Services Coordinator will update the original report and file it in the appropriate deficiency/repair binder.
III. Responsibilities:
A. All members are responsible for reporting any deficiencies of apparatus, equipment or station operations in a timely manner.
B. The Fire Services Coordinator is responsible for ensuring the Fire Chief is notified of any deficiencies reported.
C. Once approved for repair or correction, the Fire Services Coordinator is responsible for ensuring the repairs are completed by the appropriate agency.
