Postmortems
I. Scope:
This standard requires the completion of a written postmortem for all emergency incidents that involve one or more of the following:
A. A fire fatality.
B. More resources from sources outside normal firefighting.
C. A mass casualty incident.
D. A local state of emergency declared in Vegreville, that includes the department’s involvement.
E. As requested by the Fire Chief.
This standard was created to establish the guidelines for preparing and conducting a postmortem critique of a fire control or rescue incident.
II. Definitions:
A. Critique: A post incident verbal review of the problems encountered, what went right and the lessons learned during a fire control or rescue incident.
B. Postmortem: A written report published after the review of a major incident that includes all the relevant facts concerning the incident. The report shall include a review of the problems encountered, the lessons learned, a summary of the on-scene operations and any recommendations for changes in the SOG’s, etc.
III. Procedure:
A. A critique shall be conducted in but not limited to the following circumstances:
1. Fatality – civilian or department member.
2. A working structure fire – a fire that requires a considerable effort to extinguish and may require the response of additional apparatus such as tankers in rural operations.
3. Any haz-mat incident.
4. Any medical first response or EMS back-up response.
5. As deemed necessary by an officer.
B. A critique shall be conducted as soon as practical following the incident and shall include:
1. A review of the incident with all available members, generally including problems or obstacles encountered, what went well, diagram of the incident, etc.
2. Assess the need for a CISD.
3. A review of applicable SOG’s.
C. In addition to a critique a postmortem shall include but is not limited to:
1. A narrative of the incident including a summary of the operations.
2. A review of problems or obstacles encountered.
3. A review of operations that went well.
4. A diagram of the incident.
5. Recommendations for changes in SOG’s, commendations, etc.
IV. Responsibilities:
A. The incident commander shall ensure that a critique is conducted in accordance with the guidelines established by this standard. The critique shall be conducted as soon as practical following the incident.
B. The incident commander shall also have a postmortem filed and critiques conducted in accordance with this standard.
C. The incident commander shall also forward, upon request of the Fire Chief, within 72 hours of an incident a narrative of the operations conducted, including statements by any participating officer.
D. The incident commander shall forward if requested by the Fire Chief, within 72 hours of the incident:
1. A completed incident report including a narrative of the operations and statements.
2. A summary of the initial investigation
3. Reports of any and all injury and casualty reports.
4. Any relevant information deemed necessary.
